Using the Patient Portal
The Patient Portal is your practice's secure website for messages, documents, payments,
and appointments. Your practice will give you its portal address — it looks like
yourpractice.ptconn.com — and it works in any modern browser on a computer, tablet,
or phone.
Getting an account
There are two ways to get a portal account, and both verify who you are:
- Invitation — the practice sends an invitation to the email address in your patient record; the link lets you choose your own password.
- Self-registration — on the portal's sign-in page, choose Sign up, enter your email address, and confirm your identity with your date of birth. This only works when the practice has portal access enabled for your record.
Each account belongs to one patient — there are no shared or family logins, so your information is only ever visible to you.
Signing in
Sign in with your email address and password. Passwords must be at least 12 characters with an uppercase letter, a lowercase letter, and a digit. You can also add a passkey (fingerprint, face, or device PIN) for faster, safer sign-in — the portal offers this after you sign in, and you can manage it on the Security page. For your protection, the portal signs you out automatically after a period of inactivity.
If you forget your password, use Forgot password on the sign-in page to receive a reset link by email.
Messages
Send secure messages to your practice and read their replies — a private alternative to regular email for anything involving your health information. You are notified by email when the practice replies (the notification never contains the message itself).
Documents
View documents the practice shares with you, and upload documents they request — photos, forms, insurance cards, lab results. Every uploaded file is automatically converted to a safe, clean PDF before staff view it.
Your information
Review the contact and demographic information the practice has on file and submit corrections. Changes you submit are reviewed by practice staff before your record is updated.
Payments
If your practice offers online payment, you can pay balances securely by card from the portal. Card details are entered directly with the payment processor — the practice's website never sees your card number.
Appointments
See your upcoming appointments, confirm them, and — where the practice enables it — request changes. Change requests go to the practice for scheduling; you'll be contacted to finalize.
Intake forms
Practices can send you intake questionnaires to complete in the portal before your visit. Your answers go directly into your record for the practice to review, and you can stop partway and finish later.
